| Abstract | "This book provides a description of pivotal response therapy, a motivation-based approach for helping autistic children and adults develop verbal language skills while reducing interfering behaviors. Specific strategies for teaching other functions of communication are described, including question asking, commenting, play initiation and entering play, attention-seeking strategies, and assistance-seeking strategies. A randomized clinical trial comparing a PRT intervention focusing on initiations with a treatment-as-usual group showed that when intervention focused on initiations, greater gains were realized, and gains were shown in areas that were not specifically targeted, such as improvements in mean length of utterance (making longer sentences), as well as improvements in overall composites in communicative skills (i.e., speech, syntax, inappropriate initiation, coherence, stereotyped language, use of context, social relationship, rapport, and interests). Motivation is the key to improving responsiveness and correct responding, as well as decreasing disruptive and interfering behaviors. Most programs that support autistic individuals now use naturalistic procedures that include choice and other important variables to improve motivation. Pivotal response treatment has carefully defined these variables, and parents and providers can be objectively scored on their implementation of them. When PRT motivational variables are incorporated into the intervention, faster learning occurs, and learners' affect is improved with regard to happiness, enthusiasm, and interest. Parent affect in terms of happiness and interest also is improved and stress is reduced when they learn to implement the motivational procedures.
