| Summary | The intent of this study was to determine ifthe abundance of the pro-atherogenic lipoproteins common in patients with type 2 diabetes mellitus (NIDDM) is due to abnormalities in Cholesterol Ester Transfer Protein (CETP) expression. In addition, we wanted to assess the role of insulin in regulating CETP expression as a possible determinant of the increased risk of cardiovascular disease (CVD) among this population. In this study there was a slight decrease in CETP mRNA levels in adipose tissue and plasma enzyme activity in the NIDDM patients compared to lean and obese controls. This decrease was not statistically significant most likely due to the small number of NIDDM patients studied (mRNA n=4, activity n=6). Large pro-atherogenic VLDL particles were increased in the NIDDM group compared to the lean control group. This increase was statistically significant. There were no differences seen in the distribution of the LDL particles but a statistically significant increase in the large HDL particles in the NIDDM compared to the morbidly obese control group was seen. The most interesting findings were in the study of NIDDM patients following weight reduction surgery (GOP). The sub-population distribution of the VLDL particles reverted to a less atherogenic profile following weight reduction. This corresponded with dramatic improvements in insulin sensitivity and an increase in CETP activity in these patients post surgery. Although a larger NIDDM patient population is needed to show statistical significance, the following conclusions can be made from this study: improvements in insulin sensitivity of patients with NIDDM are correlated with an increase in plasma CETP activity which correspond to an improvement in the sub-population distribution in these patients. It is therefore reasonable to assume that insulin does play a role in expression of CETP that may in turn affect the composition ofthe lipoproteins.
